There are two registration forms available for printing.  The first is simply a graphic version of the registration form which you can print directly from your web browser.  The second is a PDF version of the registration form, which requires the ADOBE ACROBAT READER for viewing.

For Fairmont Hotel reservations,  use the Joint Meeting reservation form.  You must use this form to receive the special Joint Meeting rate.
